Daycare Application

The CFS 2000 is the complete application needed to submit for daycare compensation through the state of Illinois. DCFS will directly compensate the daycare provider. This application consists of numerous parts – the case management team completes a portion, the foster parent completes another and the daycare provider completes a section as well. If you need additional support in completing this form,

identifying a childcare provider or contacting the DCFS Daycare Unit, please contact us.

Social Security Request Form

This form is used for requesting the social security number for youth in your care. This is especially useful during tax season. There can be a delay in receiving the social security number from Springfield, so please be sure to allow for ample turn around time.

Visitation Supervision Form

This form is used when you are responsible for supervising a sibling, grandparent or parent child visit. This document is useful in keeping track of visitation compliance. Additionally this is a great tool for caregivers to share the appropriateness and safety of visits. There are cases where the foster parent can be compensated for supervising visits, for additional information please utilize your case management team. If they are unable to support, contact Mimi’s House and we can determine if your case meets DCFS requirements for that compensation

Comprehensive Health Evaluation Form

This is the State of Illinois form that is used for routine physicals. It is required for school, daycare and DCFS on a yearly basis. Mimi’s House strongly encourages you ask the physician to complete a vision and hearing screen at these appointments. If you need support in locating a provider in your area please contact us.

Youth Bill of Rights

This document outlines the rights for all Youth in Care. This document should be reviewed with your case management team on a yearly basis and kept in the file. Caregivers and youth will likely review it in preparation for Administrative Case Reviews (ACR). This document is a supportive tool if you feel the child in your care is not receiving everything they are entitled too.
